PLAYER REGISTRATION: (all players must be registered and forms submitted before paricipating in soccer programs)

To register a player the followiing forms must be submitted after online registration:

1. Copy of the player’s birth certificate
2. Medical Release - for players on teams that will be traveling to play in Phoenix or out of state the form must be notarized

COACH AND MANAGER REGISTRATION: (all forms must be submitted before working directly with players)

1. Online Volunteer Registraton or AYSA Admin Application
2. AYSA Volunteer Disclosure
